Guiding Patients Through Their Care
Contact Information for New Patients
Your nurse navigator will contact you, but if you need to contact them sooner, please call 206.606.1024 or submit a request for care.
A nurse navigator (NN) helps guide patients with a new or recurring diagnosis through Fred Hutch Cancer Center. Their job is to help identify what appointments you need prior to your first visit and help educate you on your diagnosis. They answer any questions you may have leading up to your initial appointment. Nurse navigators prepare you for your first and next steps at our center.
How Nurse Navigator Services Work
A nurse navigator will be your first point of contact for patients who are self-referred or referred to Fred Hutch as a new patient. Nurse navigation services are free of charge.
The nurse navigator will meet with you virtually via phone call before you are scheduled for an appointment. The nurse will ask you questions to better understand your medical, surgical, social and family history. Once the nurse has a better understanding of your diagnosis and care needs, they will connect you with our patient care coordinators (PCCs), who will schedule you for appointments as identified by the nurse.
Who Has Access to Nurse Navigation?
All newly referred patients within the following groups will be put in touch with a nurse navigator:
- Fred Hutch Cancer Center - South Lake Union: gastrointestinal cancers, genitourinary cancers, sarcoma, multiple myeloma, renal cancer, melanoma, skin cancer, breast cancer and thoracic, head and neck cancers
- Fred Hutch Cancer Center at UW Medical Center - Northwest: all cancer types
- Fred Hutch Cancer Center at Overlake Cancer Center: gastrointestinal cancers, head and neck cancers and neuroendocrine tumors
- Fred Hutch Cancer Center - Peninsula: all cancer types
- Fred Hutch Cancer Center at EvergreenHealth offers navigation services through EvergreenHealth
We are constantly working to increase the availability of nurse navigation to all patient groups at each of our patient care sites.
What You Can Expect
Your nurse navigator will provide clinical support and help troubleshoot and answer questions after your referral and leading up to your first appointment with your new provider. After your first appointment, the nurse navigator will transition your care to our clinical nurse coordinators (CNCs) to continue supporting you through your care plan.
Do I schedule nurse navigation?
You do not need to schedule an appointment with our nurse navigators. They will contact you if you are eligible.
Please allow 1-2 business days for the nurse navigator to reach out to you after your provider submits a request for care.
